Student Health Insurance Features

Outpatient Mental Health Services: $35 co-payment per visit, with a yearly maximum of $1050 for the 2002-2003 policy year.

To be initially eligible for Chickering outpatient mental health services, a student must first be evaluated at Counseling and Behavioral Health Services (CBH). If appropriate, CBH will fill out an Insurance Authorization Form and submit it to the insurance company. It is then the student's responsibility to submit a pink claim form (available at CBH) and the first bill to the Chickering insurance company at address cited above.

To be reauthorized for outpatient mental health services for the following year, a student must have the private therapist fill out a Mental Health Evaluation Form (available at CBH) and mail it to Chickering.

Outpatient Alcohol & Substance Abuse Services: 60 outpatient visits per calendar year, 20 of which can be applied toward group therapy.

Inpatient Hospitalization: $5,000 per condition (basic plan) or $10,000 (comprehensive)

Partial Hospitalization: $5,000 per condition (basic) or $10,000 (comprehensive)
